The awarded shares are subject to time-based vesting after 48 months and are subject to forfeiture until vested.; On December 15, 2025, Intelligent Bio Solutions Inc. (the "Issuer") effected a 1-for-10 reverse stock split of the Issuer's common stock ("Common Stock"). The reverse stock split effected in December 2025, and prior reverse stock splits effected by the Issuer, resulted in a reduction in the number of shares held by the reporting person and proportional adjustments to the outstanding equity awards held by the reporting person. Accordingly, all amounts in this Form 4 are presented on a post-reverse split basis.
